The Dog House Day Care Membership Form

To help us better understand you and your dogs needs, please indicate the type of services you are interested in. These details are used as part of our assessment / marketing and does not commit you to any services at The Dog House

Evaluations are conducted prior to your dog attending daycare, on a day that suits you best. The assessment which takes a minimum of half an hour is to determine suitability.

The first day of daycare is on a trial basis at full cost.

I understand I must pay for any daycare in advance using one of our concessions or on the day.
The full daycare cost will be applied irrespective of how long your dog is in daycare (we do not have half day rates).
The Dog House concessions are valid for two months from the date of purchase, please use them in time to avoid disappointment.
I understand I must give notice of cancellation or change of schedule day/time for grooming appointments at least one full business day before and that I will be charged 100% for "no shows" if the appointment cannot be filled.
I understand the hours of operation are 7:15 am to 5:30 pm. Early drop off from 7 am can be arranged prior to the day for an additional $10 fee. Late pick up will be charged at $10 for every half hour (or part of) after close at 5.30pm.
I understand that after 6:30 pm overnight boarding charges will be applied (Overnight cost plus next day daycare charges).

I (the owner), hereby declare to The Dog House that I am the legal owner of my dog; that my dog has not been exposed to distemper, rabies or parvo within the past 30 days; that my dog has been inoculated as indicated by records presented and that my dog is currently and properly licenced.
I understand that The Dog House is a kennel-free facility. I accept the potential risks involved and agree that The Dog House is not liable for any injuries or illnesses resulting during my dogs attendance. Dogs play using teeth and paws (claws).
I understand if my dog has a history of aggression or biting, The Dog House reserves the right to refuse entry to my dog and I have disclosed to The Dog House all known dangers associated with my dog.
I understand I am liable for any medical care expenses and damages that result from injuries caused by my dog.
I expressly waive and relinquish any and all claims against The Dog House, its employees and representatives, except those arising from negligence on the part of The Dog House.
I understand that under no circumstances will The Dog House be liable for consequential damages or damages beyond the replacement value of my dog.
If any medical problems develop while my dog is in the care of The Dog House, I autherise The Dog House to do whatever is deemed necessary for the safety, health and wellbeing of my dog, and I agree to assume full financial responsibility for any and all expenses incurred.
I understand if my dog comes into The Dog House and has fleas I will be charged for a flea bath (min $25).
I understand if my dog is in for a groom and has to be shaved close to the skin because of matting, they may have a reaction to the clippers or be left with marks where the matts were tight to the skin. I am aware the shortest blade can easily cut the skin.
I understand that we value "humanity over vanity" so we will not subject dogs to painful dematting when shaving is recommended.
I understand The Dog House has the right to refuse service to me or my dog at any time and for any reason.
I understand by allowing my dog to participate in services offered by The Dog House I authorise the taking of photographs of my dog for publication and/or use in promotions.
